Concord, MA, January 24, 2025 – Deaconess Abundant Life Services (DALS) is proud to announce our partnership with Care Dimensions as part of the Centers for Medicare and Medicaid Services program, Guiding and Improved Dementia Experience (GUIDE). GUIDE services aim to improve the quality of life for those living with dementia, reduce strain on unpaid caregivers, and enable those living with dementia to remain in their homes and communities as long as possible.
Highlights of this program include:
- Services are covered under Medicare at no additional cost to the patient or caregiver.
- Comprehensive assessments and care plans.
- Care coordination with the patient’s existing healthcare provider.
- Resources for unpaid caregivers including training and emotional support.
A person is eligible to receive services under the GUIDE program if they meet the following criteria, as verified by Medicare:
- Has mild, moderate, or severe dementia, as confirmed by one or the Care Dimensions physicians or nurse practitioners.
- Is enrolled in Medicare Parts A and B (not enrolled in Medicare Advantage, including Special Needs Plans, or PACE programs) and has Medicare as their primary payer.
- Is not enrolled in hospice.
- Is not a long-term nursing home resident.
